Massachusetts Resident Open Enrollment
Most Massachusetts residents who are age 18 or older must now have health insurance or pay a tax penalty. Massachusetts makes available high-quality, affordable health insurance through the Health Connector. Commonwealth Choice health insurance options are available to you on a payroll deduction, pre-tax basis. Your participation is optional.
Open Enrollment into the state system is June 1, 2013 through June 30, 2013 for an August 2, 2013 effective date. You will have received direct notification of this Open Enrollment from Favorite.

Therefore, upon hire, you will be requested to complete the Employee Health Insurance Responsibility Disclosure Form (Employee HIRD Form) by the personnel in your branch office.
